Our Intake Department has a Part-time opening for an  Intake Coordinator

 to join our dynamic team at our Corporate Headquarters with the potential to work from  home after 6 months.

The Intake Coordinator

acts as a liaison between clients and families, the Agency, member hospitals/clinics and outside agencies.  Provides services at quality levels consistent with professional standards of practice.  Assists internal and external customers with questions regarding pay source regulations and about specific Home Care needs. 

Collects data about patient’s medical condition, psychosocial history, physical limitations, etc., from the referring agency and/or the patients’ medical record.

Interviews client, family and/or significant other in order to assess all of the patients needs.  Collaborates with client, family and/or significant other to coordinate admission to services.

Seeks physician and healthcare staff input in selecting the most appropriate services for the patient.

Investigates patient eligibility for program services with payer, determines patient eligibility for program services based on governmental requirements, and informs patient of findings and financial responsibilities. Insurance verification form is completed on a thorough, accurate and timely manner.

Communicates information to healthcare team in a timely and courteous manner.  Communicates changes in admission information to family and healthcare team.  Collaborates and cooperates with  Agency staff in planning care for clients with exceptional needs.

Serves as a liaison between the client and family, the Agency, member hospitals/clinics and outside agencies, by practicing the principles of customer service.

Acts as a resource and provides educational programs for internal/external customers regarding Home Care programs, benefits and eligibility requirements. 

Assists in meeting the Agencies annual goals by referring an established number of referrals per month for each Agency program.

Assists Manager in training staff for Intake/Home Care Coordinator positions.

Aides in coverage for Home Care or Hospice Coordinators as needed.
