The Field Care Manager, Behavioral Health 2 (Care Manager, Behavioral Health) assesses and evaluates member's needs and requirements to achieve and/or maintain optimal wellness state by guiding members/families toward and facilitate interaction with resources appropriate for the care and wellbeing of members. The Field Care Manager, Behavioral Health 2 (Care Manager, Behavioral Health) work assignments are varied and frequently require interpretation and independent determination of the appropriate courses of action. 
Responsibilities
The Field Care Manager, Behavioral Health 2 (Care Manager, Behavioral Health) employs a variety of strategies, approaches and techniques to manage a member's physical, environmental and psycho-social health issues. Identifies and resolves barriers that hinder effective care.
Performs telephonic and face to face assessments and evaluations of the member's needs and requirements to achieve and/or maintain an optimal wellness state by guiding members/families toward the appropriate resources for the care and overall wellbeing of the member.  
Ensures member is progressing towards desired outcomes by continuously monitoring care through assessments and/or evaluations.
Creates member care plans. Understands department, segment, and organizational strategy and operating objectives, including their linkages to related areas.
Collaborates with providers and community services to promote quality and cost-effective outcomes. 
Required Qualifications
Applicable State licensure in field of study: Licensed Masters Clinical Social Worker (LCSW), Clinical Social Worker (CSW, Licensed Professional Counselor (LPC) Registered Nurse (RN) Licensed Marriage Family Therapist (LMFT) or other professional license.
Health professionals with a degree in nursing must have at least two (2) years practice experience in a behavioral-health field or setting.
Minimum of 1 year of post-degree clinical experience in behavioral health setting.
This role is considered member facing and is part of Humana's Tuberculosis (TB) screening program. If selected for this role, you will be required to be screened for TB.
This role is part of Humana's Driver safety program and therefore requires an individual to have a valid state driver's license and proof of personal vehicle liability insurance with at least 100/300/100 limits.
Must have a separate room with a locked door that can be used as a home office, to ensure you have absolute and continuous privacy while you work.
Must have the ability to provide a high speed DSL or cable modem for a home office (Satellite and Wireless Internet service is NOT allowed for this role). A minimum standard speed for optimal performance of 10x1 (10mbs download x 1mbs upload) is required.
Must be passionate about contributing to an organization focused on continuously improving consumer experiences .
Must reside in the state of Oklahoma; travel within the state is expected to meet with members.
Preferred Qualifications
Master's Degree with Corresponding Masters Prepared Licensure.
Experience working with Medicare, Medicaid and dual-eligible populations.
Case Management Certification (CCM).
Field Case Management Experience.
Bilingual preferred (Spanish, Native Tribal, Vietnamese or other).

, within the state of Oklahoma to meet with members and occasional travel to our Oklahoma City office location for staff meetings, training, etc.

If participating in a pre-recorded interview, you will respond to a set of interview questions via your phone. You should anticipate this interview to take approximately 10-15 minutes.
If participating in a SMS Text interview, you will be asked a series of questions to which you will be using your cell phone to answer the questions provided. Expect this type of interview to last anywhere from 5-10 minutes. Your recorded interview(s) via text and/or pre-recorded voice will be reviewed and you will subsequently be informed if you will be moving forward to next round of interviews.
