The Associate Director of Care Team Nursing requires an in-depth understanding of how organization capabilities interrelate across the function or segment.

The Associate Director of Care Team Nursing requires an in-depth understanding of how organization capabilities interrelate across the function or segment.
The Associate Director of Care Team Nursing employs a variety of strategies, approaches, and techniques to manage a member's physical, environmental, and psycho-social health issues. Identifies and resolves barriers that hinder effective care. Ensures member is progressing towards desired outcomes by continuously monitoring member care through assessments and/or evaluations. May create member care plans. Decisions are typically related to the implementation of new/updated programs or large-scale projects for the function and supporting technical/operational procedures and processes, and implements strategic plans, drives goals and objectives, and improves performance. Provides input into functions strategy.
Job Description:
The Associate Director of Care Team Nursing for Author by Humana will serve as the nursing leader within an interdisciplinary team providing whole person care for seniors with multiple chronic conditions and unmet social needs. This is a unique opportunity to help lead a team transforming the care that we provide for seniors - working to meet their social, emotional and medical needs - and help shape the necessary tech-enabled services to deliver on that experience from the ground up.
The role requires experience caring for patients with complex medical and social needs and an understanding of how to provide care more efficiently and effectively with an interdisciplinary team. We are looking for individuals who are passionate about transforming our care system for patients who need it most and are driven by doing well by doing what's right. This candidate must be an insightful risk-taker willing to thoughtfully disrupt the status quo and excited to work in a rapidly evolving start-up environment
The role of Associate Director of Care Team Nursing will focus on the following areas:
*Serve as a thought leader in designing and then implementing the role of nurses on our interdisciplinary care team
*Develop and monitor all clinical protocols, either independently or in collaboration with other interdisciplinary care team members as relevant, for nurses on care team to use for both in person and virtual (both tech-enabled and telephonic) care management, chronic disease management and care at points of transition
*Determine state dependent nursing requirements and modify work of the care team as necessary to ensure compliance with all local policies and practices
*Oversees the hiring and management of a team of nurses in the market that provide direct member care.
*Work to identity what tech-enabled services will be necessary for team to provide necessary care and work with product team on development
Required Qualifications
Bachelor's Degree
6 or more years of technical experience
5 or more years of management experience
Active Registered Nurse or Nurse Practitioner licensure and ability to obtain appropriate licensure in our target market to provide direct patient care and oversight for team for team as necessary.
Must be passionate about contributing to an organization focused on continuously improving consumer experiences.
5 years of experience in clinical setting taking care of patients with multiple chronic conditions including but not limited to hypertension, diabetes, COPD and heart failure
Experience in practicing in ambulatory care settings, providing in home care and care management experience.
Computer proficiency with knowledge of MS Office products, specifically Word, Excel, and Outlook
Experience with electronic health record documentation/digital health programs
Expected travel 1-2x per month
Preferred Qualifications
Master's Degree
Experience in disease management and wellness programs
Previous Medicare/Medicaid Experience a plus
