Responsibilities

Conviva is seeking an LPN/LVN , who will assess patient health problems and needs, takes vital signs, administers medication, develops, and implements nursing care plans, and maintains medical records. May advise patients on health maintenance and disease prevention or provide case management.

Job Functions:


Works closely with the clinic care coach to provide clinical education and guidance to patients
Works closely with primary care doctors to serve patients by providing disease screening, administering immunizations
Records patient statistics
Participating in educational opportunities
Obtains health history and/or immunization status by interviewing patient and/or relative


Requirements:


Active LPN license
IV Certified
1-2 years' experience as nurse in inpatient or outpatient clinic setting
Multi-tasking and organization skills
Bilingual preferred


Working hours : Monday to Friday 8AM-5PM
