The Floating Telehealth Psych Nurse Practitioner provides care across several programs to ensure programs have appropriate coverage.u003c/pu003enu003cpu003eu003cstrongu003eQUALIFICATIONSu003c/strongu003eu003c/pu003enu003cpu003eu003cstrongu003eRequired: u003c/strongu003eu003c/pu003enu003culu003enu003cliu003eBacheloru0026rsquo;s Degree in Nursing Science and an advanced degree in a healthcare science as required by the State Licensing programu003c/liu003enu003cliu003eCompletion of an accredited Nurse Practitioner programu003c/liu003enu003cliu003eANCC Board Certified as a Psychiatric Mental Health Nurse Practitioner (PMHNP)u003c/liu003enu003cliu003eLicensure as an Advanced Practice Nurse Practitioner in the State of practiceu003c/liu003enu003cliu003eActive DEA and Furnishing licenseu003c/liu003enu003cliu003eMust be at least 18 years of ageu003c/liu003enu003cliu003eMust be CPR, Crisis Prevention Institute (CPI), and First Aid certified on date of employment or within 60 days of employment and maintain current certification throughout employmentu003c/liu003enu003cliu003eAll opportunities at Telecare are contingent upon successful completion and receipt of acceptable results of the applicable post-offer physical examination, 2-step PPD test for tuberculosis, acceptable criminal background clearances, excluded party sanctions, and degree or license verification. Additional regulatory, contractual or local requirements may applyu003c/liu003enu003c/ulu003enu003cpu003eu003cstrongu003ePreferred:u003c/strongu003eu003c/pu003enu003culu003enu003cliu003eMinimum of one (1) year experience working as a Psychiatric Mental Health Nurse Practitioner with persons with serious mental illnessu003c/liu003enu003cliu003ePrevious experience working in acute settings, such as a Psychiatric Health Facility or Evaluation and Treatment Centeru003c/liu003enu003cliu003ePrevious experience evaluating and treating persons with serious mental illness through telehealthu003c/liu003enu003c/ulu003enu003cpu003eu003cstrongu003eESSENTIAL FUNCTIONSu003c/strongu003eu003c/pu003enu003culu003enu003cliu003eDemonstrate the Telecare mission, purpose, values and beliefs in everyday language and contact with the internal and external stakeholdersu003c/liu003enu003c/ulu003enu003cpu003eu003cstrongu003eAssessment and Diagnosisu003c/strongu003e:u003c/pu003enu003culu003enu003cliu003eInterviews members served to gather relevant history including, but not limited to, current psychiatric symptoms, previous psychiatric treatment and hospitalizations, previous response to medications, substance use, physical health issues, history of abuse or neglect, history of violence, and pertinent family historyu003c/liu003enu003cliu003eUtilizes effective communication methods to foster the therapeutic relationship and promote positive clinical outcomesu003c/liu003enu003cliu003eGathers data from multiple sources including reviews of previous medical records, psychosocial and other evaluations performed by multidisciplinary team, and interviews family members as appropriateu003c/liu003enu003cliu003eUsing data gathered, completes comprehensive psychiatric evaluations of members served within designated timeframes; the Tele-Psychiatric evaluation must include content areas as determined by Practice Site policies as well as regulatory and Telecare standardsu003c/liu003enu003cliu003eDevelops a diagnosis based on the data gathered in accordance with ICD-10/DSM-V classification systemsu003c/liu003enu003cliu003eEvaluates need for involuntary hold status and release as appropriateu003c/liu003enu003cliu003eProvides necessary legal documentation for involuntary status members served.u003c/liu003enu003c/ulu003enu003cpu003eu003cstrongu003eTreatment:u003c/strongu003eu003c/pu003enu003culu003enu003cliu003eDevelops plan for treatment based on data gathered during assessment and input from members served as well as other sources; ensures that the plan for treatment is in accordance with evidence-based standards and practice guidelinesu003c/liu003enu003cliu003eWorks in collaboration with multi-disciplinary team to implement plan for treatment; participate in interdisciplinary treatment team meetings. The team may include various disciplines including but not limited to mental health therapists, Nurse Practitioners, Physician Assistants, psychologists, occupational therapists, speech therapists, dietitians, substance recovery counselors, wellness coaches, and educatorsu003c/liu003enu003cliu003eOrders further diagnostic and laboratory testingu003c/liu003enu003cliu003eCoordinates care with other consulting providersu003c/liu003enu003cliu003ePrescribes psychiatric medications based on clinical indicators and results of diagnostic and laboratory testing as appropriate; monitors and documents members served response to medications and makes adjustments as appropriateu003c/liu003enu003cliu003eEducates members served and families about the benefits and adverse effects of prescribed treatment options.u003c/liu003enu003cliu003eMonitors/reviews members served progress in treatment in collaboration with the multidisciplinary treatment team.
