Responsibilities
Conviva Care Solutions is seeking a hard-working and dedicated Triage Nurse
to promote and restore patients' health by completing the nursing process and who is interested in being part of a team that focuses on excellent service to others.

Essential Functions
Identifies patient care requirements by establishing personal rapport with patients Establishes a compassionate environment by providing emotional, psychological, and spiritual support to patients, friends, and families.
Responsible for l evaluation of patients with ambulatory diagnoses as well as possible acute conditions. The triage nurse will work in partnership with the center's mid-levels and PCPs.
In consultation with the center's mid-levels and / or PCPs the triage nurse will initiate treatment if necessary, for common ambulatory diagnoses.
Promotes patient's independence by establishing patient care goals; teaching patient, friends, and family to understand condition, medications, and self-care skills; answering questions.
Assures quality of care by adhering to therapeutic standards; measuring health outcomes against patient care goals and standards; making or recommending necessary adjustments; following hospital and nursing division's philosophies and standards of care set by state board of nursing, state nurse practice act, and other governing agency regulations.
Resolves patient problems and needs by utilizing multidisciplinary team strategies.
Work in other duties as assigned
Position Requirements:
Must be willing and able to perform the following procedures or at least be willing to be trained to do them:
Starting and IV and initiating IV fluids
Give Injections
Simple wound care
Insert a foley catheter if appropriate
Administer aerosol treatments
Perform EKGs
Good communication skills
Works well in a team of healthcare providers.
Education:
Associates Degree or Bachelor of Science Degree in Nursing
Nursing Diploma
Working hours
: Monday to Friday 8AM-5PM
Additional Information
This role is considered patient facing and is part of Humana/Senior Bridge's Tuberculosis (TB) screening program. If selected for this role, you will be required to be screened for TB.
For this job, associates are required to be fully COVID vaccinated or undergo weekly COVID testing and wear a face covering while at work. The weekly testing will need to be done through an approved Humana vendor, and unvaccinated associates should follow all social distancing and masking protocols if they are required to come into a Humana facility or work outside of their home.
