We have an opportunity to join the Alliance as the Care Coordination Supervisor (RN or MSW) in the Care Coordination Department.
This position can be filled in any of the Alliance offices: Salinas, Scotts Valley or Merced.
WHAT YOU'LL BE RESPONSIBLE FOR
Supervise, mentor and train assigned staff
Supervise, coordinate and implement effective and efficient Case Management and Care Coordination programs and processes, including Complex Case Management, Medical Social Work, Enhanced Care Management/Community Support, and Care Coordination, act as a subject matter expert, and provides guidance on unit functions
ABOUT THE TEAM
Coordinates care and empowers members to achieve their health and wellness goals through collaboration and by facilitating member relationships with community providers

Knowledge of:
Principles and practices of clinical nursing or social work, as applicable to license type
Care management and coordination
Principles and practices of supervision and training
Principles and practices of case management
The healthcare industry, managed care, and the Medi-Cal program and related policy
Ability to:
Evaluate medical records and other health care data
Supervise, train, mentor, and evaluate the work of staff
Motivate staff to achieve goals and objectives, and promote an atmosphere of teamwork and cooperation
Plan, organize and prioritize tasks and work schedules, manage projects, adhere to timelines, delegate effectively, and follow up on work assignments
Act as a technical resource and explain regulations, processes and programs related to area of responsibility
Work independently, apply sound judgment, and make decisions related to areas of functional responsibility
Education and Experience:
RN position:
Current unrestricted license as a Registered Nurse issued by the state of California
Associate’s degree in Nursing and a minimum of seven years of experience in a patient care setting, including at least one year of case management experience and one year of supervisory or lead experience (a Bachelor’s degree may substitute for two years of the required experience); or an equivalent combination of education and experience may be qualifying
MSW position:
Master’s degree in Social Work and a minimum of three years of experience in a client or patient care setting, including at least one year of case management experience and one year of supervisory or lead experience; or an equivalent combination of education and experience may be qualifying
OTHER DETAILS
While this position is connected to one of our Alliance offices, we are in hybrid remote/in-office work environment right now and we anticipate that the interview process will take place remotely.
Our Alliance office locations have officially re-opened as of May 2, 2022 and while some employees may work in full-time telecommute schedules, attendance at quarterly company-wide events or department meetings will be expected.
Based on the nature of work, this position may require onsite presence, which is dependent on business need. Details about this can be reviewed during the interview process.
