The Caregiver at Care Partners is the most important member of the Care Partners Team. Working 1-on-1 with Care Partners clients and families, our Caregivers bring love, empathy and compassion to our clients by assisting seniors with important activities of daily living (ADLs) as well as companionship and support.

What We Can Provide For You:

Once You Are Hired, Here’s What Working with Care Partners Looks Like:
1. You will lead first by demonstrating an understanding of the Care Partners At Home mission: We will create deep meaningful relationships with our clients, their families, our associates, and partners resulting in the establishment of high energy environments focused on true person centered caring, while increasing the quality of their lives, and ours, within every interaction.
2. You will engage your client with the purpose of understanding their unique needs so as to best represent, advocate and provide feedback to both the organization and family when appropriate.
4. You will provide basic Personal Care Attendant services to clients as identified by Care Partners in the client’s Daily Care Plan. Typical activities you will assist with include:
· Dressing, bathing, toileting and grooming assistance
· Meal preparation and feeding assistance
· Assistance with light housework
· Companionship and games
· Walking and transfer assistance
· Assistance and supervision of physical therapy exercises
· Errand and Dr.’s appointment accompaniment
5. You will be prepare and maintain accurate and timely client notes, documenting the tasks you performed with the client, any client issues or any positive client interactions
Job Requirements
Our ideal candidate starts with the following:
· At least 18 years old
· Previous experience as a Caregiver or recent graduate from Home Health Aide or Certified Nursing Assistant program
· Ability to read and write English
· A current state driver’s license or government-issued identification
· Access to reliable transportation
· Ability to pass a medical physical as well as provide written proof of health status
· A positive and caring attitude
· A willingness to learn and grow
· The ability to dress neatly and represent Care Partners in a professional way
