POSITION SUMMARY:
Coordinate and facilitate access to medical care for IHH clients experiencing complex acute and chronic illnesses. Eligible clients continue to need CSP-level mental health services and require frequent medical specialist appointments, medication adjustments, in-home nursing services, and other interventions to maintain lowest level of care for medical need. Duties include medication reconciliation, close coordination with a dedicated case manager, coordination with assigned IHH Team, appointment and transportation scheduling, occasional appointment attendance, written and telephonic coordination with all providers.
EDUCATION AND TRAINING REQUIRED:
· RN required; bachelor’s degree preferred. Current license to practice in Rhode Island.
· Experience and/or interest in working with individuals with chronic psychiatric impairment and/or chronic medical conditions and/or substance addictions
· Current CPR/AED certification
· Desire and ability to work in a fast-paced cooperative, multi-disciplinary team environment
· Employee will assume full responsibilities for the cost and efforts of maintaining any and all forms of licensure, certification and credentialing documentation as required in their job description or cited on any Action Form as a qualification for hiring, job promotion, or monetary increase. Subsequently, the employee will adhere to the credentialing requirements of all insurance/funding sources for which they qualify and provide Human Resources and/or the Credentialing Specialist with the initial required documentation (licenses, liability insurance policies, DEA numbers etc.) and all renewals of these documents
WORKING CONDITIONS:
· Must be able to read, write, speak and comprehend English.
· Must possess fine and gross motor skills required to handle individual and packaged medications, assess blood pressure and vital signs, and administer IM injections
· Must be able to maintain concentration and organization and complete assigned tasks while working in close quarters with others.
· Must possess sound judgment under duress and communicate effectively with members of a variety of professional disciplines in complex circumstances.
· Must have access to and be willing to transport clients in a safe, insured vehicle.
