Many of the innovative programs and vital services that we offer are not available elsewhere.We are searching for an Admissions Coordinator to join our team.This position is based out of our St. Margaretu2019s Center in Albany, NY.u00A0 St. Margaretu2019s Center is a 94 bed skilled nursing facility providing short-term, long-term, and respite care to children and young adults with disabilities.u00A0 Our dedicated team of professionals delivers individualized care focusing on the unique needs of our residents.u00A0 To learn more about St. Margaretu2019s Center, please check out our Virtual TourJob Summary:The Admissions Coordinator leads the networking and admissions process with the goal of maintaining 100% occupancy with accompanying waitlist. Locates and assess potential residents for appropriate St. Margaretu2019s Center placements. Works with hospital discharge planners, case managers, residents/families, and other referral sources to expedite placement and admissions to the facility.Essential Job Responsibilities: Effectively markets services delivered by St. Margaretu2019s Center to hospitals, prospective residents, families, social service localities, insurance companies, service coordination providers and, the general public Networks with Hospitals/Community: serves as the designated staff member for strengthening communication and marketing services to the community Maintains routine contact via phone, email, mailings, and in-person with referral sources throughout NY and neighboring states (includes potential for air travel) Maintains competitive advantage through regular assessments and strategize competitor facilitiesu2019 marketing activities and census trends Provides clinical assessment of all applicants for admission; makes recommendation to Executive Director, Medical Director and DON on appropriate fit for permanent and respite placement Manages the admission process for new residents into the facility including completing necessary documentation. Collaborates with Social Worker and other IDT members to ensure that the comprehensive intake process is completed Verifies payer sources and collaborates with Benefits Specialist that payer source are current and valid Leads Admissions process including meeting with new residents and their families to facilitate a smooth transition including, accurate and comprehensive completion of Admissions agreements, consents, insurance and other required documentation Facilitates Pre and Post-Admission Team meetings to evaluate the admission processQualifications: Current and valid NYS License as a Licensed Practical Nurse or Registered Nurse PRI Credentialed (If Licensed as a Registered Nurse) PASRR Certification u2013 preferred upon hire; required for the position Current and valid NYS Driver License At least 2 years of job related experience, specifically practicing as a LPN or RN in a skilled nursing facility; pediatric experience preferredAt The Center for Disability Services, we donu2019t just accept difference u2013 we celebrate it, support it, and thrive on it for the benefit of our employees and the people we support.
