Home Care Assistance is looking for an LPN to lead as Client Care Manager on our team!
We are seeking highly motivated, customer service-oriented superstars to join us in our mission to Change the Way the World Ages. We welcome applications from RNs and LPNs to play a key role in enhancing the quality of life for our senior clients and delivering an outstanding client experience.
The Client Care Manager assists clients and their families with decisions related to aging, available services, and long-term care planning. You will conduct in-home care consultations with prospective clients, build trusting relationships and convert them to Home Care Assistance clients. You will develop and oversee a (non-OASIS) plan of care, supervise in-home care providers, and maintain an ongoing relationship with clients and their stakeholders to help ensure the best care possible.
The ideal candidate is passionate about helping seniors, confident working in a consultative capacity, has a strong customer service orientation, is thorough with details, and adept with computer systems and mobile technology.
Job Requirements:
Graduate of an accredited school of nursing with two year of clinical experience within the past five years or completion of an accredited refresher course within the past two years
Current Oklahoma RN or LPN license
Available for work on weekends and evenings
Meets current health requirements to provide patient care
Ability to read, write, speak and understand English as needed for the job
Possess a valid driver's license and have use of an insured automobile
Clean criminal background
Minimum of 2 professional references
Travel in OKC metro area required. Company car or mileage reimbursement provided.
Applicants must be flexible to work evenings and weekends.
