The RN Case Manager will collaborate closely with other disciplines including social workers, providers, utilization review, and ancillary departments. Through this collaboration, and the use of care guidelines, the RN Case Manager helps to ensure progression along clinical pathways and is involved in creating a safe transition into post-acute settings. This particular RN Case Manager opening is for our Jones 11 Orthopedic unit. The RN Case Manager represents Virginia Mason with professionalism when interacting with community agencies and facilities. Bachelors of Science in Nursing
Minimum of 3 years of experience in care coordination, care management, discharge planning, or relevant/related experience
Current Washington State Registered Nurse License
Preferred:
Master’s degree in NursingExperience in Orthopedics and other Medical/Surgical nursing
