The Role

Be a part of the Clinical Care Team-- as a Care Coach you will work alongside a PCP in a clinic to engage our members to develop lifelong well-being and health. Conviva Care Solutions is seeking a Care Coach to help coordinate and evaluate the management of patients with acute and chronic conditions, as well as patients discharged from acute facilities, to achieve high quality care. You will be an integral part of clinical rounds and interact with all levels of nursing and other departments to assure effective utilization of resources that meet the physiological and safety needs of the patient and their families.

Basic clinical knowledge and experience at a level sufficient to read, understand and present inpatient medical records and patient's current condition

Demonstrated ability to create an exceptional patient experience leveraging excellent customer service skills

Excellent communication skills.

Actively listen and ensures understanding regardless of the communication vehicle; understand the needs and perspectives of others and tailors care coordination activities accordingly; share information appropriately

Interpersonal effectiveness (emotional intelligence): Understand oneself, effectively manage emotions, listen and communicates with respect, and build trusting relationships

Ability to work well with cross functional teams; Develop and manage interpersonal relationships with members of the entire care team

Demonstrated ability to interview and educate patients while understanding their basic clinical needs

Responsibility for achieving organizational goals and accountability for results. Meet established expectations and take accountability for results; encourage others to do the same

Strong organization and time management skills with the ability to prioritize and multi-task.

Organize and manage multiple priorities and/or projects using appropriate methodologies and tools

Self-motivated and committed to achieving best results

Possess critical thinking skills, independent decision making capability

Implement physician approved care plans for patients in the Care Coordination System.

Educate patients from a standard education engagement plan

Record patient care and progress on appropriate documents including updating patient-specific care plans. Maintain and properly log patient follow up calls as required

Educate and teach patients and families regarding patient disease and procedures that will directly affect the patient under the supervision/direction of a registered nurse or physician.

Flexible to changes in work schedule and location as needed by the organization

Maintain standards for professional nursing practice and demonstrate the ability to use the nursing process in setting

Vaccine Policy

Humana and its subsidiaries require vaccinated associates who work outside of their home to submit proof of vaccination, including COVID-19 boosters.

Associates who remain unvaccinated must either undergo weekly negative COVID testing OR wear a mask at all times while in a Humana facility or while working in the field.

Role Requirements:

Active LPN/LVN License

Active CPR-BLS from the American Heart Association

Minimum of 2 years of prior clinical experience in an acute or post-acute setting

Knowledge of community health and social service agencies and additional community resources

Proficiency with computers including Microsoft products, Word, Excel, and Outlook

Able to provide face to face and telephonic patient care

This is a hybrid role; will travel to PCP offices and local corporate office 3-4 days/week. Will have the ability to work at home when possible.

Preferred Requirements:

A minimum of of two years managed care experience and/or discharge planning experience

Experience with Electronic Medical Records; e-Clinical Works (eCW)

Bilingual - English, Spanish

CD-10, CPT codes, HCPCS knowledge preferred

Managed care experience including knowledge of HEDIS and CMS guidelines

Additional Information

Hours: 8-5 / M-F

Local weekly travel to PCP Offices and Corpus Christi Conviva Corporate Office

Occasional travel to San Antonio for conferences

This role is considered patient facing and is part of Humana At Home's Tuberculosis (TB) screening program. If selected for this role, you will be required to be screened for TB.

Hybrid Office Requirements

This is a hybrid role; the incumbent will travel to PCP offices and local corporate office 3-4 days/week and has the ability to work at home when possible

This role will require local travel to see members within a 30 miles radius from where you reside. Travel could be to a members home, PCP Office, Local Hospital or other clinical settings

WAH requirements: Must have the ability to provide a high speed DSL or cable modem for a home office. Associates or contractors who live and work from home in the state of California will be provided payment for their internet expense.

A minimum standard speed for optimal performance of 25x10 (25mpbs download x 10mpbs upload) is required.

Satellite and Wireless Internet service is NOT allowed for this role.

A dedicated space lacking ongoing interruptions to protect member PHI / HIPAA information
