Description
The Care Manager, Telephonic Nurse, in a telephonic environment, assesses and evaluates members' needs and requirements to achieve and/or maintain optimal wellness state by guiding members/families toward and facilitate interaction with resources appropriate for the care and wellbeing of members. The Care Manager, Telephonic Nurse work assignments are varied and frequently require interpretation and independent determination of the appropriate courses of action.
The Care Manager, Telephonic Nurse employs a variety of strategies, approaches and techniques to manage a member's physical, environmental and psychosocial health issues. Identifies and resolves barriers that hinder effective care. Ensures patient is progressing towards desired outcomes by continuously monitoring patient care through assessments and/or evaluations. May create member care plans. Understands department, segment, and organizational strategy and operating objectives, including their linkages to related areas. Makes decisions regarding own work methods, occasionally in ambiguous situations, and requires minimal direction and receives guidance where needed. Follows established guidelines/procedures.

Required Qualifications
Active Registered Nurse (R.N.) license from any state by examination
If you do not have an Active RN license from passing the NCLEX you must have an Active Register Nurse License from Puerto Rico and meet one of the following criteria:
2 consecutive years of experience as RN of the preceding 3 years (no gaps in employment) and apply for FL RN license endorsement as soon as employment offer is accepted. OR
Graduated from an Accreditation Commission for Education in Nursing (ACEN) or Commission on Collegiate Nursing Education (CCNE) accredited nursing program, must successfully complete and pass the NCLEX exam within 60 days of employment
Other Requirements:
Demonstrated clinical knowledge and experience with adult chronic conditions
Intermediate computer skills and experience with Microsoft Word and Excel
Proficient typing and computer navigation skills
Capacity to multi-task including use of multiple computer applications simultaneously
Effective communication and interpersonal skills
Effective problem solving and appropriate application of clinical knowledge
Able to work 40 hours and adjust work schedule according to business need including occasional weekends
Bilingual in Spanish and English. Must be able to speak, read, and write in both languages without limitation or assistance. See Additional Information about testing
Preferred Qualifications
Bachelor's (BSN) or Master's (MSN) degree in nursing
Successful completion of National Council Licensure Examination (NCLEX)
Previous managed care experience
Basic knowledge of complex care management and care management principles
Experience with motivational interviewing
Experience with behavior change, health promotion, coaching and wellness
