This position has responsibility for providing direction, planning, implementation, evaluation and consultation to ensure that Performance Management, Education, Patient Safety and Quality metrics meet and/or exceed state and national benchmarks across the L M Healthcare Delivery network-including LMH, WH and Pequot campuses.

This involves working to improve key processes, using data driven metrics and evidence-based practice in a collaborative manner with key stakeholders across the organization aligned with the mission, vision, values and strategic goals of the organization. She/he has responsibility to coordinate activities with Professional Development Specialists, Nursing leaders and Providers on performance improvement projects and staff education. She/he utilizes performance management and Six Sigma tools to achieve service line and hospital initiatives and goals. This work requires extensive knowledge, skills and expertise in change and performance management. This position will lead a diverse group of staff skilled in assisting the organization in achieving success in mission critical initiatives through project management, process improvement, consultation, facilitation and collaboration.

1. Under the direction of the Administrative Director, manages PI plan implementation:

1.1 Develops the annual Patient Care Services PI plan with input from stakeholders and effectuates such plan over the course of the fiscal year

2. Under the direction of the Administrative Director, develops and manages Patient Safety/Quality Goals

2.1 Develops timetables and action plans to complete patient safety/quality goals and projects

3. Under the direction of Nursing Leadership, oversees data reporting for performance management requirements to all regulatory and accrediting bodies.

3.1 Facilitates and serves on quality and safety committees to assure regulatory and accrediting body readiness in terms of data collection, benchmarking and action planning for hospital accreditation.

4. Serves as the main contact for assigning, directing and managing quality improvement, patient safety activities for Patient Care Services

5. Serves as the facilitator and/or leader of quality improvement and safety projects

6. Maintains a relevant knowledge base for an ever-changing landscape of evidence-based practice and regulatory readiness through concurrent training, quality and safety peer outreach and literature review.

7. Performs other assignments as required and/or directed by Nursing Leadership

8. Meets performance expectations for Customer Service, Teamwork, Resource Utilization, and Staff and Self Development as outlined in performance review.

Qualifications

EDUCATION

Baccalaureate of Sciences in Nursing required. Master?s degree in Nursing, Nursing Administration, Patient safety, Business, Public Health or related field with an emphasis in health care management strongly preferred

EXPERIENCE

Five to seven years? experience in health care environment, including a minimum of three (3) years? experience in project management, research and evaluation of healthcare outcomes and three (3) to five (5) years of management/supervisory experience.

LICENSURE

Active CT Registered Nurse License Required

SPECIAL SKILLS

Excellent interpersonal, communication and leadership skills. Ability to work collaboratively and independently; demonstrated personal maturity and good judgement. Expertise in data collection and process improvement techniques. Expert knowledge of information systems and external databases associated with accreditation and regulatory requirements for quality of care, treatment and service evaluation and public reporting. Special training Performance Improvement methodologies.

Accountability for planning, implementation and evaluation of departmental specific quality and safety performance improvement. In personal and job-related decisions and actions, consistently demonstrates the values of integrity (doing the right thing), patient-centered (putting patients and families first), respect (valuing all people and embracing all differences), accountability (being responsible and taking action), and compassion (being empathetic).
