Description
The Care Manager, Telephonic Nurse 2 , in a telephonic environment, assesses and evaluates members' needs and requirements to achieve and/or maintain optimal wellness state by guiding members/families toward and facilitate interaction with resources appropriate for the care and wellbeing of members. The Care Manager, Telephonic Nurse 2 work assignments are varied and frequently require interpretation and independent determination of the appropriate courses of action.
Responsibilities
The Care Manager, Telephonic Nurse 2
Employs a variety of strategies, approaches and techniques to manage a member's physical, environmental and psycho-social health issues.
Identifies and resolves barriers that hinder effective care
Ensures patient is progressing towards desired outcomes by continuously monitoring patient care through assessments and/or evaluations
Create member care plans
Understands department, segment, and organizational strategy and operating objectives, including their linkages to related areas
Makes decisions regarding own work methods, occasionally in ambiguous situations, and requires minimal direction and receives guidance where needed
Follows established guidelines/procedures.
Occasional travel (less than 20%, see additional information below)
Monday - Friday 10:30am-7pm
Required Qualifications
Unrestricted Licensed Registered Nurse (RN) in the Florida with no disciplinary action
5 years of clinical OB Maternity and Women's Health care experience
Comprehensive knowledge of Microsoft Office applications including Word, Excel, and Outlook
Must be passionate about contributing to an organization focused on continuously improving consumer experiences
Additional Requirements
Hours: Monday - Friday 10:30am-7pm EST and be flexible due to the business needs
Work style: This position is considered remote/work at home
Location: Tampa, FL
Travel: To local Market office based on business needs to attend meetings and occasional travel to provider offices (See Additional Information)
Remote Work at Home Requirements:
Must ensure absolute and continuous privacy while you work within a home office or other remote location.
Must have accessibility to hardwired high speed internet with minimum speeds of 10Mx1M for a home office (Wireless and Satellite are prohibited)

Possession of a valid driver's license and access to a reliable automobile
This role is part of Humana's Driver Safety program and therefore requires an individual to have a valid state driver's license and proof of personal vehicle liability insurance with at least 100,000/300,000/100,000 limits.
TBA and AHCA
This role is considered patient facing and is a part of Humana's Tuberculosis (TB) screening program. If selected for this role, you will be required to be screened for TB.
AHCA Fingerprint Required.
Preferred Qualifications
Experience with case management, discharge planning and patient education for adult acute care
Knowledge of Milliman or Interqual
Managed Care experience
Certified Case Manager (CCM)
Bilingual
