JOB SUMMARY

Carilion Clinic Hospice Services is seeking a Hospice Nurse for our Roanoke office located at 1615 Franklin Road in Roanoke.  This position is full time with flexible start/end times to accommodate department needs and will work independently in a variety of home settings.

This position will support Carilion's hallmarks of service excellence and quality for all job responsibilities. Will serve as a member of the Hospice team and serves as a case manager for assigned patients. Coordinates symptom management for assigned patients.  Coordinates care of patient/family unit in collaboration with members of hospice IDT team, Medical Director and attending physician.

MINIMUM QUALIFICATIONS

EDUCATION: Must be a graduate of an approved school of professional nursing/RN program.

EXPERIENCE: Must have a mimimum of one year experience in hospice, home health, oncology, or medical surgical nursing.

LICENSURE/CERTIFICATION: Must have a current valid Virginia RN license.  Must have valid driver's license, automobile insurance and personal vehicle for making home visits.

OTHER: Must have a clear criminal records check as approved through the Virginia State Police criminal record exchange and found to be  free from barrier crimes.
