Summary and Scope
At the direction of registered nurses and providers, the LPN provides direct patient care in an acute care setting utilizing various nursing skills in accordance with patient needs, established standards of care and organizational policies.
Essential Functions:
POLICIES AND PROCEDURES - Maintain established departmental policies and procedures, objectives, and quality assurance programs
PROFESSIONAL DEVELOPMENT - Enhance professional growth and development through participation in educational programs, reading current literature, attending in-services, meetings and workshops.
Adheres to the LPN scope of practice by contributing to the assessment of the health status of designated patients.
Under the direction and co-signature of registered nurses, the LPN may assess and document nursing assessment, plan of care, interventions, evaluation and re-evaluation of patient status.
Deliver direct patient care, assist nurses and providers with various procedures including administering treatments as prescribed. Documenting individual patient needs, interventions and patient outcomes.
Under the direction of registered nurses, the LPN promotes patient’s independence by supporting patient care goals; teaching patient, friends, identified lay caregiver and family to understand condition, medications, and self-care skills; answering questions.
As prescribed by physician or standard protocols, administer medications, including mediations given through established parenteral/intravenous infusions. Document according to policy.
Under the direction of registered nurses, the LPN may assess and document changes in patient’s condition and report to charge nurse and/or nurse and physician as needed.
Maintain current certifications for appropriate specialty and keep current with developments in the field.
Report potential and actual patient safety, concerns, medical errors and or near misses in a timely manner thru the chain of command and document within the PSN system.
Establish a compassionate environment by providing emotional psychological and spiritual support to patients, and their friends and families.
Active participation in hospital strategic initiatives such as quality improvement and monitoring TJC compliance.
Monitor and initiate corrective action to maintain the environment of care including equipment and material resources.
Maintain a cooperative relationship among health care teams and resolve patient problems and needs by utilizing multidisciplinary team strategies.
Able to document and communicate pertinent information using computer and/or paper documentation tools in a timely and accurate manner.
Perform miscellaneous job-related duties as assigned.

Knowledge of scope of the Licensed Practice Nurse (LPN)
Education/Experience
Education:
High School Diploma or GED
Licensed Practical Nursing Formal Education from an accredited program
Licenses/Certifications:
Licensed Practical Nurse-NM
CPR Certification or must obtain within thirty (30) calendar days of date of position
Basic Arrhythmia Certification or equivalent within one (1) year of date of position
ACLS certification or must obtain within in six (6) months of date of position
Complete and maintain unit based required certifications and competencies as listed in the department expectations and or the unit education matrix
Complete and maintain SRMC IV certification competency within sixty (60) calendar days of date of position
Complete and maintain SRMC Medication Administration competency within sixty (60) calendar days of date of position
Preferred Educational/Experience Requirements:
Licensed Practical Nursing degree from an accredited institution
Six (6) months of recent (within the last year) experience in Hospital, Long Term Care/Nursing Home setting
Dual Language Certification
Conditions of Employment
Must pass a pre-employment criminal background check, reference checks and a post offer drug screen.
Must be employment eligible as verified by the U.S. Dept. of Health and Human Services Office of Inspector General (OIG) and the Government Services Administration (GSA).
