Description
The Telephonic Care Manager Nurse 2 assesses and evaluates members' needs and requirements to achieve and/or maintain optimal wellness state by guiding members/families toward and facilitate interaction with resources appropriate for the care and wellbeing of members. The Telephonic Care Manager Nurse 2 work assignments are varied and frequently require interpretation and independent determination of the appropriate courses of action.
Responsibilities
The Telephonic Care Manager Nurse 2 employs a variety of strategies, approaches and techniques to manage a member's physical, environmental and psycho-social health issues.
Identifies and resolves barriers that hinder effective care.
Ensures patient is progressing towards desired outcomes by continuously monitoring patient care through assessments and/or evaluations.
May create member care plans.
Understands department, segment, and organizational strategy and operating objectives, including their linkages to related areas.
Makes decisions regarding own work methods, occasionally in ambiguous situations, and requires minimal direction and receives guidance where needed.
Follows established guidelines/procedures.
Must reside in Florida
Required Qualifications
Unrestricted Licensed Registered Nurse (RN) in the state of Florida with no disciplinary action
Certified Diabetes Care and Education Specialist (CDCES)
3 - 5 years of clinical acute care experience
Comprehensive knowledge of Microsoft Office applications including Word, Excel, and Outlook
Must be passionate about contributing to an organization focused on continuously improving consumer experiences
Schedule: 8 hour shift Monday-Friday 8:30 AM-5:00 PM Eastern Time.
Travel: attend meetings in market office as needed, once restrictions are lifted (See additional Information) Travel requirements will be discussed during the interview
Training: 6-8 weeks in WAH office, virtually
Screening Requirements:
Associate working in the State of Florida will need Completion of a Level II; AHCA background clearance Fingerprinting.
Work at Home/Remote Requirements
Must have a separate room for privacy with a locked door during working hours and during communication with members.
WAH requirements: Must have the ability to provide a high-speed DSL or cable modem for a home office (Satellite and Wireless Internet service is NOT allowed for this role). A minimum standard speed for optimal performance of 10x1 (10mbs download x 1mbs upload) is required. A dedicated space lacking ongoing interruptions to protect member PHI / HIPAA information
Preferred Qualifications
Experience with case management, discharge planning and patient education for adult acute care
Knowledge of Milliman or Interqual
Managed Care experience, including Medicaid Managed Care
Certified Case Manager (CCM)
Bilingual in Spanish Preferred
BSN Preferred
